The midface is usually the first area of the face to show meaningful volume loss — cheeks flatten, the under-eye area becomes more hollow, and the overall facial shape shifts from a youthful upward-pointing triangle toward something flatter and more descended. Restoring volume here has a disproportionate effect on the rest of the face: the nasolabial folds soften, the lower face looks less heavy, and the overall contour re-establishes itself.
Dr. Martin places filler deep, close to the bone, where it provides structural lift rather than soft tissue fullness — which is what produces a natural result rather than an augmented one. Overfilling the cheeks is one of the most common errors in aesthetic medicine. Dr. Martin doses conservatively, assesses the result, and adds incrementally if warranted — because adding more at a follow-up is straightforward, and managing an overfilled result is not. Hyaluronic acid fillers are used throughout for their reversibility. Downtime is one to three days with mild swelling and possible bruising.
How does volume loss in the midface affect the overall appearance of the face?
The midface is the structural foundation of the lower two-thirds of the face — when it loses volume, everything below it is affected. Cheeks flatten, the under-eye area becomes more hollow, nasolabial folds deepen, and the face loses the inverted-triangle shape associated with youth. Many patients notice they look tired or aged without being able to identify exactly why — and midface volume loss is often the reason. At our practice, we assess the midface in relation to the full face rather than in isolation, because restoring volume in the right location has a cascading effect on everything below it.
How do you determine the right amount of filler to use?
The right amount is whatever achieves subtle, natural-looking restoration without overcorrection — and that requires restraint. Overfilling the cheeks is one of the most common aesthetic errors we see corrected at our practice, and it tends to result from placing too much volume without adequate attention to the patient's underlying bone structure and natural proportions. We use a conservative, layered approach — placing less than we think might be needed, assessing the result, and adding incrementally if warranted. It's far easier to add more at a follow-up than to manage an overfilled result.
How long does midface filler last?
Cheek filler placed near the bone tends to last longer than filler placed in more superficial or mobile areas — typically twelve to eighteen months, and sometimes longer depending on the product and the patient. Because the midface sees relatively little movement compared to the lips or lower face, filler in this region tends to be metabolized more slowly. At our practice, we discuss expected longevity at consultation and build a realistic maintenance schedule into the long-term plan so patients aren't surprised by when retreatment becomes appropriate.
Is cheek filler right for everyone?
Not necessarily — and we'll tell you honestly if we think it isn't the right approach for you. For patients with significant structural bone loss or significant skin laxity, filler alone may not provide the support needed for a natural result, and we may discuss alternative options or a surgical consultation. For patients who are very young and have no meaningful volume loss, adding filler to the cheeks can actually look unnatural — a fuller midface may not reflect their natural anatomy.
