Lip Lines

Fine vertical lines above the upper lip form from a combination of orbicularis oris muscle activity, collagen loss in the perioral skin, and sun exposure accumulated over time. They're among the harder lines to treat well because this is one of the highest-movement areas of the face — filler metabolizes faster here and requires more precise placement than in less mobile regions.

Dr. Martin addresses them through a combination approach: superficial filler along the lines softens their depth and improves the lip border; perioral neurotoxin reduces the muscle activity that drives their continued formation between sessions. Neither alone is a complete answer. For patients with significant perioral wrinkling, laser resurfacing — the ECO2 or Sciton Halo — is what produces the most durable structural improvement, and Dr. Martin often coordinates injectables with laser in a sequenced plan rather than treating them as separate concerns. Downtime is one to two days for filler; the neurotoxin component has no recovery. The combined result is a meaningfully refreshed perioral area without the telltale signs of isolated filler placement.

Why do lip lines form, and why are they so hard to treat?

Perioral lines — the fine vertical lines above the upper lip — form from a combination of repetitive orbicularis oris muscle activity, progressive collagen loss in the perioral skin, and sun damage over time. What makes them challenging is the high degree of movement in this region — the orbicularis oris is one of the most active muscles in the face, and any filler placed here is metabolized faster and requires more precise placement than in less mobile areas. At our practice, we're upfront that perioral lines are one of the more difficult concerns to fully resolve, and we often recommend a combination of injectables and laser to achieve the best result.

What does a combination approach look like for lip lines?

For most patients with established perioral lines, the most effective plan combines filler to soften existing line depth with perioral neurotoxin to reduce the muscle activity that drives their continued formation — and laser resurfacing to address the underlying collagen loss in the skin itself. Filler and neurotoxin can improve the lines significantly, but they can't stimulate new collagen in the perioral skin. For that, the ECO2 fractional CO2 laser or the Sciton Halo Tribrid is the most effective tool we have. At our practice, we discuss all three modalities at consultation and recommend based on the severity of the lines and the patient's tolerance for downtime.

Will filler make my lips look overfilled if it's used for lip lines?

Not when placed correctly. The filler used for perioral lines is placed superficially along the line itself — in very small amounts — to soften the crease, not to add volume to the lip. This is distinct from lip augmentation filler, which is placed within the lip body. At our practice, we're precise about the distinction and about using the right amount for the right purpose — the goal is to soften lines subtly, not to change the shape or apparent size of the lip as a side effect.

Is there anything I can do to prevent lip lines from worsening?

Several things. Daily broad-spectrum SPF — including directly on and around the lips — addresses one of the primary accelerants of perioral skin aging. A retinol-based product applied carefully to the perioral area stimulates collagen and can slow the development of new lines over time. Staying well hydrated and avoiding smoking are also meaningful contributors. At our practice, we include skincare guidance as part of every treatment consultation — because what you do at home between appointments genuinely influences how your results hold up and how slowly the lines progress.
