Nasolabial Folds
Nasolabial folds and marionette lines deepen as midface volume depletes and soft tissue descends — they're a downstream consequence of structural changes higher in the face rather than a problem originating in the fold itself. Treating them by filling the crease directly often produces a result that looks corrected rather than rejuvenated.
Dr. Martin's approach addresses the underlying cause: restoring volume in the midface first, which allows the fold to soften naturally as the tissue regains structural support from above. When filler in the fold itself is warranted, it's placed conservatively and in the context of what's already been done higher up. Marionette lines are addressed with targeted placement at the prejowl and along the line, calibrated to the degree of descent and the patient's overall facial structure. Downtime is one to three days with mild swelling and possible bruising. Results typically last nine to twelve months, though the midface tends to hold filler longer than the lower face due to reduced movement in that area.
What causes nasolabial folds and marionette lines to deepen?
Both are primarily driven by midface volume loss and soft tissue descent with age. As the fat pads in the cheeks deplete and the supporting ligaments loosen, the overlying tissue descends — and the skin folds more deeply at the nasolabial crease and along the marionette region. Genetics play a role in how prominently these lines develop, and sun damage accelerates the collagen loss that reduces the skin's ability to resist folding. At our practice, we view these lines as a downstream consequence of midface structural changes rather than an isolated surface concern — which shapes how we approach treatment.
Why don't you just fill the fold directly?
You can, and sometimes it's part of the plan — but filling the fold in isolation often produces a result that looks corrected rather than rejuvenated. The fold deepens because tissue above it has descended, so adding volume directly into the fold addresses the surface expression of the problem without correcting the underlying cause. By restoring volume higher in the midface first — in the cheeks and along the cheekbones — the fold softens more naturally as the tissue regains support from above. At our practice, we approach this structurally rather than cosmetically, and the results look more natural and last longer as a result.
How long does filler in this area last?
Nasolabial and marionette filler typically lasts nine to twelve months, with some products lasting longer depending on their formulation and the patient's individual metabolism. The marionette region tends to hold filler slightly longer than the nasolabial fold because there's less movement in that area. At our practice, we choose products based on the specific area being treated — softer, more malleable formulations for areas with movement, and more structured products for areas where support is the priority. Is filler for nasolabial folds safe?
Hyaluronic acid filler placed by an experienced provider in this area has a well-established safety profile. The nasolabial region is less vascular than some other areas of the face — like the nose or glabella — which reduces the risk of vascular complications. That said, no injectable treatment is entirely without risk, and proper technique matters. At our practice, every filler treatment is performed with a thorough anatomical assessment, conservative volumes, and the knowledge that hyaluronidase is immediately available to dissolve the product if any concern arises.
