Pigmentation
& Melasma

Pigmentation concerns range from isolated brown spots to melasma — and those two things require meaningfully different approaches. Isolated spots from sun exposure or post-inflammatory marks respond well to targeted laser energy.

Melasma is more complicated: it's hormonally influenced, prone to recurrence, and can worsen with aggressive treatment if the skin is overstimulated. Dr. Martin takes a deliberately conservative, progressive approach to melasma — the goal is steady reduction without triggering a rebound, which is a real risk if the treatment is too intense too soon. The Spectra MDF and Hollywood Spectra platforms use Q-switched Nd:YAG energy to shatter melanin deposits selectively, scheduled as a series rather than a single aggressive session. The Lase MD Ultra gently exfoliates the superficial layers where much of the discoloration lives and also supports the delivery of topical brightening agents into the skin post-treatment. For patients with photodamage alongside pigmentation, the Sciton Halo Tribrid addresses both in one session. Patients with melasma hear directly from Dr. Martin that ongoing sun protection and maintenance treatments are part of any realistic plan — because the condition is managed, not cured.

What's the difference between melasma and regular pigmentation?

It's an important distinction because the two require different treatment approaches. Regular pigmentation — from sun exposure, post-inflammatory marks, or isolated spots — is relatively straightforward to address with targeted laser energy. Melasma is more complex because it has a hormonal component, which means it's prone to recurrence and can actually worsen if treated too aggressively. At our practice, we take a much more conservative, progressive approach to melasma than we would to other pigmentation concerns — the goal is steady improvement without triggering a rebound, which is a real risk with overly aggressive treatment.

Why does melasma keep coming back after treatment?

Because treatment doesn't remove the underlying cause — it manages the expression of it. Melasma is driven by melanocytes that are hypersensitive to hormonal fluctuations and UV exposure, and those melanocytes remain active even after the visible pigmentation has improved. Without consistent sun protection and maintenance, the pigmentation returns. At our practice, we're honest about this upfront: melasma is a condition you manage, not one you cure. The patients who do best are the ones who understand that ongoing sun protection and periodic maintenance sessions are part of the long-term plan.

How does laser treatment help with pigmentation without making it worse?

The key is selecting the right laser energy and using it at the right intensity for the specific type of pigmentation being treated. Our Spectra MDF and Hollywood Spectra platforms use Q-switched Nd:YAG energy that selectively targets melanin deposits without generating the heat that can inadvertently stimulate more melanin production — a phenomenon called post-inflammatory hyperpigmentation. At our practice, we sequence treatments conservatively and monitor skin response session by session. For melasma in particular, we often start with the gentlest effective approach and only escalate if the skin is responding well.

Are there things I should avoid that make pigmentation worse?

Several things — and they're worth knowing regardless of whether you're in treatment. Unprotected sun exposure is the primary driver of almost all pigmentation concerns, so daily broad-spectrum SPF is the single most important habit to build. For melasma specifically, hormonal fluctuations from oral contraceptives, pregnancy, or hormone replacement therapy can trigger or worsen the condition regardless of how well the skin is being treated. Heat exposure — from hot yoga, saunas, or even very hot showers — can also stimulate melanocytes and drive melasma in susceptible patients.
