Surgical, Trauma
& Stretch Mark Scars
Scars from surgery, trauma, or stretch marks share the same underlying problem: collagen architecture was disrupted during healing, leaving textural and pigmentary irregularities that don't resolve on their own. Significant improvement is achievable — but the treatment has to match the scar.
For raised, depressed, or irregular textural scars, the ECO2 fractional CO2 laser delivers ablative energy through precise microcolumns, stimulating new collagen formation within the scar tissue over five to seven days of recovery. For scars that are primarily pigmented or superficially irregular, the Sciton Halo Tribrid addresses both surface texture and discoloration in a single session with a three to five day recovery window. Genius RF Microneedling works beneath the scar surface to remodel the underlying collagen without disrupting the epidermis — minimal downtime, and a useful adjunct that Dr. Martin often sequences alongside laser work to extend and deepen results. Treatment plans for significant scarring are built across multiple sessions, adjusted based on how the tissue responds.
Can laser treatment really improve the appearance of scars?
Significantly, yes — though the degree of improvement depends on the scar's age, type, and depth. Laser treatment works by stimulating the skin to produce new collagen within and around the scar tissue, gradually replacing disorganized scar collagen with more organized, functional collagen that better resembles normal skin. Textural improvement, softening of raised scars, and reduction of pigmentation are all achievable outcomes. At our practice, we're always honest that no treatment erases a scar entirely — but meaningful improvement in color, texture, and surface irregularity is consistently achievable with the right approach.
What's the best treatment for surgical scars versus stretch marks?
Both benefit from fractional laser resurfacing, but the specific approach varies. Surgical scars that are raised, thickened, or irregular in texture respond well to the ECO2 fractional CO2 laser, which stimulates deep collagen remodeling and has a strong track record with hypertrophic scarring. Stretch marks — which are essentially atrophic scars — also respond to the ECO2 and to Genius RF Microneedling, which rebuilds the collagen and elastin beneath the scar surface without disrupting the epidermis. At our practice, the Sciton Halo Tribrid is often used for scars with both a pigmentary and textural component, as it addresses both layers in a single session.
When is the best time to start treating a scar?
For surgical and trauma scars, earlier intervention — once the wound has fully closed and the initial healing phase is complete — tends to produce better long-term outcomes than waiting years for the scar to mature. Starting treatment in the early remodeling phase, typically three to six months after injury or surgery, allows us to influence the collagen organization while the tissue is still actively rebuilding. Older, established scars can also be improved, though they may require more sessions. At our practice, we assess scar maturity and vascularity before recommending a start point, because treating too early carries its own risks.
How many sessions will I need, and what is the downtime?
The number of sessions depends on the scar's severity and how it responds to initial treatment — most patients see meaningful improvement over two to four sessions, though significant scarring may require more. For the ECO2 fractional CO2 laser, expect five to seven days of recovery. The Sciton Halo Tribrid involves three to five days of downtime. Genius RF Microneedling is the most recovery-friendly option at one to two days of mild redness. At our practice, we sequence treatment based on the scar type and the patient's schedule, often combining modalities within an overall plan to maximize results.
